   You may add your photograph to this screen if
   you have created a .BMP, .PCX or .GIF file of
   your picture from a scanner or other source that
   provides this type of service. The required
   action is that you must rename the picture file
   with the last 6 numbers of your actual Social
   Security number and use the extension .PCX and
   copy the renamed file into the FMHM directory.
   The filename to be used will be displayed at the
   bottom of the photo frame for the member being
   processed. 
   Note: Default install directory is C:\MEDHIS
   rename example:

   This report gives the user the flexibility of
   selecting all or a single family member. Leave
   SSN field as 0 to select all family members.
   Enter the SSN of a single member to print the
   Visitations report for only that member. The
   report selection can be further tailored to
   select only one or all visit types for the
   single SSN entered or all members. Use the
   choice button to select the desired visit type.
   You may also use any combination of the above
   criteria with the report beginning and ending
   period selections to tailor this report even
   further. Use any desired starting and ending
   dates; the program will select only visiation
   records for visit dates within the period.
   Leave as is to select all visit dates within
   program's database. Note that the period end
   date defaults to the current system date and may
   be overwritten to reflect desired ending date.

<Immunizations and Shots Report>

   This report will list Immunization records for
   all the family members. This report also gives
   the user the flexibility of selecting all or a
   single family member. Leave SSN field as 0 to
   select all family members. Enter the SSN of a
   single member to print the Immunizations report
   for only that member. You may also use any
   combinations of the above criteria with the
   report beginning and ending period selections to
   tailor this report even further. Use any desired
   start and ending dates; the program will select
   only records for immunizations received within
   the period. Leave as is to select all of the
   immunization dates within program's database.
   Note that the period end date defaults to the
   current system date and may be overwritten to
   reflect desired ending date.

   system for family members. This report also
   has the flexibility of selecting all or a

   above criteria with the report beginning and
   ending period selections to tailor this report
   even further. Use any desired start and end
   date; the program will select only medication
   records with begin dates within the period.
